Medical Records Please be assured that all medical records remain confidential. Previous treatment records should be sent to your Primary Care Physician prior to your first visit. This will ensure that your physician is familiar with previous conditions and treatment and that medical decisions are made based upon your medical history as well as your current medical condition. If you need a copy of your medical record please contact the physician’s office to request a copy.
