Specialty care If your Primary Care Physician feels that you have a health need that requires diagnosis or treatment from a specialist, he/she will coordinate ALL referral care that you receive by initiating a referral form. Most services do not require review of Managed Health Care Associates and the referral form will be given to you by your PCP, WPHCP, or Specialist. Services that require the review of Managed Health Care Associates are sent to the administrative office for review. You should receive your referral form in the mail within 5 business days. No appointment(s) should be made prior to receiving an approved referral. If you do not receive a referral form in 5 days, contact our office at 773-271-0880. In the event your referral is denied you will be notified of the reason for the denial, the alternative treatment, a telephone number for questions, and the mechanism for appeals will be communicated to you in writing.
