Referrals

Your Primary Care Physician is responsible for:

• Referring you to an appropriate medical specialist.


• Monitoring the specialist’s treatment.


• Authorizing any further specialist care that you require beyond the initial referral

YOU are responsible for:

• Not scheduling an appointment without an authorized referral form.


• Being aware of the expiration date of the referral, the type of service and the number of visits the referral authorizes.


• The co-payment that will be due at the time of service.

Please Remember:

• Referrals are issued ONLY to providers contracted with Managed Health Care Associates.


• All referrals are for ONE VISIT ONLY unless otherwise specified.


• Do not schedule an appointment until you receive your copy of the authorized referral form.


• You must bring your copy of the referral form with you to your appointment and be prepared to pay your co-payment.
